Requesting a COVID-19 Test
Currently, East Valley is only testing East Valley patients
and those without insurance. Thank you!
Due to an increased demand for COVID-19 testing and to better serve our patients please email us at covidtesting@evchc.org to schedule your COVID-19 test. We will need basic information to help you secure your appointment. Please include your Full Name, Date of Birth, primary contact number and insurance information (if any) for a representative to connect with you as soon as possible.

Necesitaremos información básica para ayudarle a asegurar su cita. Incluya su nombre completo, fecha de nacimiento, número de contacto principal e información de seguro (si existe) para que un representante se conecte con usted tan pronto como sea posible.
